Date of
Eligibility: Health coverage will begin
on the 1st day of the month following 1 month of service. EX: Date of hire: 11/25/03 Eligible: 01/01/04.

Receive cash payments
directly. Full policy benefits
regardless of any other insurance.
Personal control over benefit dollars to help cover leftover medical
expenses, living costs, and loss of income.
Individual ownership – portable and guaranteed-renewable at the same
payroll rate.

Date of
Eligibility: Health coverage will begin

2.
Two types of
deductibles paid per calendar year
a) $150 deductible per enrollee and $450/family.
b) $50 Emergency Room deductible per hospital emergency
room visit and is waived if admitted as an inpatient, or accidental injury and
treatment is given within two days after the accident.
3. Health Benefit Payments: (Are made after deductibles are satisfied.)
a) Health benefits to a lifetime maximum of $1,000,000
for covered services and
Supplies at 80%/20%.
b) 80% of $2,500 in allowable charges for covered
services received by enrollee in a calendar year and 100% coverage after that.
c) Dental coverage is 70% the first year for basic
coverage, 80% - 2 years, 90% - 3 years and 100% - 4 years.
Fortis
is the Life Insurance, Short and Long-Term Disability and Accidental Death
Dismemberment provider.

Date of Eligibility:
Life, AD&D and Disability coverage will begin on the 1st day of the month following 1 month of
service. EX: Start Date: 11/25/03 Eligible:
01/01/04.
2. Life Insurance Benefit is equal to two ½ times the
employee’s annual regular earnings to a minimum of $50,000 to a maximum of
$250,000.
3. Accidental Death and Dismemberment Benefit is equal
to two times an employees annual regular earnings to a maximum of $100,000.
4. Short-Term Disability is for employees who incurred
an injury, illness or pregnancy. The first 7 days (including weekends),
employee use flex/extended illness/LWOP and disability pay starts on the 8th
day. Weekly disability is calculated
at 60% of employee’s gross earnings to a maximum of $500 per week. Maximum benefit period is 26 weeks.
5. Long-Term Disability is for employees who have
incurred a disability that goes beyond the 26-week recovery period. Long-Term disability is calculated at 60% of
employee’s regular earnings to a maximum of $5,000 per month.
6. Funeral Benefit is $1000 to cover funeral costs in
the event of a death of an eligible employee.
1. Date of Eligibility: The Money
Purchase Plan (Retirement Plan) is for employees who completed a 6-month
waiting period. Eligible employees
enter the plan on the 1st day of the quarter following completion of
a 6-month waiting period (Jan. 1st, April 1st, July 1st
& Oct. 1st).
2. Contributions of 7% of
employee’s annual earnings, are made by the Corporation. An employee becomes 25% vested after 3
years, 50% vested after 4 years and 100% vested after 5 years of service.
Vesting is calculated after 12/31 of each year.
3. A loan program is available
once a person is 25% or more vested. The employee can borrow up to 50% of their
vested account balance or a minimum of $1000.00.

The MPP TSA is
available to eligible employees on the date of hire.
2.
Employees can put up to
25% of their salary to a cap of $11,000 into TSA accounts.
3.
TSAs are
pre-taxed. Monies are deducted from
employee’s bi-weekly salary before taxes are taken out.
4.
Employees are 100%
vested with TSA.
2. These plans are pre-tax,
long-term savings.
3. Employee pays the premiums/costs and automatic
bi-weekly payroll deductions are placed into a TSA account of choice.
NOME
RECREATION CENTER/NOME BELTZ HIGH SCHOOL SWIMMING POOL
1. Date of Eligibility:
Date of hire.
2. HR issues a picture ID and employee shows ID to the
Nome Recreation Center/NBHS Swimming Pool desk clerk. This benefit is for employees ONLY.
