BRIEF SUMMARY
 OF JOB:

To perform comprehensive needs assessments of Family Care Givers and dependent family members. Acts as a liaison between multiple agencies and ensures that informal Family Caregivers and dependent family member have access to all appropriate medical, social, behavioral health, educational and ancillary services necessary to assist the caregiver to provide adequate, appropriate care to the dependent family member, while taking care of their own medical, psychological and social needs.
QUALIFICATION REQUIREMENTS: Education: High School diploma or GED is required. Registration/Certification: None Required
Experience: 6 months experience working with Elders in home and community based services.
Skills: Requires good organizational skill with attention to detail. Must be able to communicate effectively (oral and written). Possess ability to follow detailed written and oral instruction. Demonstrate professionalism, consideration and confidentiality towards others in stressful situations. Basic Computer skills are preferred.
WORK SCHEDULE: Monday through Friday, 8 a.m to 1 p.m.
