Diabetes Prevention/Healthy Living Grant

The NSHC Diabetes Prevention and Chronic Care Program is pleased to announce a non-competitive grant program to help prevent diabetes in the villages of Norton Sound. We will be accepting proposals for projects that promote healthy eating, physical fitness, and traditional lifestyles in the villages of Norton Sound.

Guidelines: Proposals should represent cooperative efforts between IRA councils, city councils, and village health care providers. Projects may include organized community activities, educational programs, exercise equipment, incentive programs, or any activity that will encourage healthy living by community members. Applications should include the title of the proposed project and a brief narrative description, including who the project will benefit, who is responsible for organizing the project, and how long the project will continue. A preliminary budget should also be included. Please use the attached application form for proposals.

Monies for these grants are available to NSHC as part of a Diabetes Prevention Grant from the Special Diabetes Program for Indians and Native Alaskans. Up to $15,000 can be made available to each village that submits proposals meeting the above guidelines. Grants to villages will be renewable annually, provided the federal monies for diabetes prevention continues to be available to NSHC.

Special Diabetes Program For Indians (SDPI)

The non-competitive diabetes grant provides funds for the CAMP department to offer diabetes management and prevention services. Activities included in this program include:
  • Diabetes management – Certified Diabetes Educator Rita Binford and Dietitian Kelly Keyes work to help those with diabetes manage this condition to prevent complications.
  • Step by Step – make an appointment to have your Total Cholesterol, HDL, LDL, Triglycerides, Blood Sugar, Blood Pressure, Weight and Waist Circumference checked and make goals to improve or maintain your health. A Rec Center and Swimming Pool pass and pedometer may be available to participants.
  • Tribal Prevention Program – Village IRA Councils can apply for up to $15,000 to provide diabetes prevention activities and programs in the community.
  • Community Presentations – CAMP staff are available to provide presentations on request.
  • Summercise – two 3-week sessions of exercise, nutrition and fun! Kids ages 5 and up learn about nutrition and exercise while having fun. Local leaders and dietary interns lead the activities while teaching kids about eating healthy and fun ways to be active.
SDPI Diabetes Prevention Program (DPP)
This grant provides funds for the CAMP department to coordinate a research-based diabetes prevention program to those that are high-risk for diabetes. The intensive portion of this program includes a 16 session curriculum and regular check-ins. The non-intensive portion allows for community based activities such as:
  • Water Aerobics – An aerobic program at the Nome Swimming Pool as scheduled. An instructor stands on the deck and leads movements for those in the water. Participants can work at any pace, increasing resistance with gloves and other accessories at the pool. Low impact workout, decreasing stress on joints.
  • Park And Walk: People Realizing it’s Important Not To Sit (PAW PRINTS) – walk to work, school, on your breaks, anytime. This program encourages walking at least 30 minutes each day to earn points. Prizes are available at certain checkpoints.
  • Community Health Fair – hospital departments and community programs are invited to have a booth at the annual health fair in Nome. Health fair packets and guidance from CAMP staff are available to each village that would like to provide their own health fair.
Health Promotion/Disease Prevention (HP/DP) – Tobacco Cessation and Prevention
The grant has three goals which include:
  • Goal #1- Prevent smoking and sustain abstinence in youth through school-based education and community outreach
  • Goal #2- Reduce the number of adult smokers and sustain abstinence through hospital-based smoking cessation programs
  • Goal #3- Promote a smoke-free environment through community partnerships and Public Policy. The accomplishments for each goal and objective are summarized below.
