A Strategic Planning Retreat was held on Friday, November 1st at the Aurora Inn. The retreat was extremely successful and included representatives from various programs and consumers.  An overall Mission as well as Three goals were developed:

Mission:

Empowering people and communities to promote healthy lifestyles and improve the quality of life for people with chronic illnesses

Case Management, Support and Self-Care:

To personally empower individuals to take ownership of their healthcare through family and community based support to improve the quality of life for people with chronic illnesses.

Community Education and Screening:

To empower communities to develop culturally sensitive programs promoting health lifestyles to prevent chronic illnesses

Program Management:

To support the chronic care/diabetes prevention team by integrating information, encouraging teamwork, and securing resources.
